Health Information

All new students must submit their health information. 

All incoming students to the Qatar campus are required to have a complete medical file.  This includes completing a medical history form and uploading vaccine/immunization records from their healthcare providers before arriving on campus. 

We strongly encourage you to read the information carefully and ask questions to the Director of Health and Wellness early because gathering all the required documents can be a timely process.  All documents for incoming students are due on July 1st for the Fall semester and December 31st for Spring semester.

Submit Health Information using Health Connect

  • You will need your Andrew ID and Password (once your student account is created (this is typically done once you have paid your deposit), you will receive an email on how to find your Andrew ID and set your initial password)
  • Access Health Connect
  • Log in with your Andrew ID and password
  • Enter your date of birth
  • Use the left menu to access forms and complete all the required forms:
    1. Health History Form
    2. Immunization Form
    3. Immunization/Vaccination Requirements: Ensure that you receive all the required immunizations and provide the appropriate evidence.  You can use the Vaccination/Immunization Form to verify that you have met all the requirements OR your doctor can sign this form verifying that all vaccinations/immunizations and screenings are complete.
    4. Notice of Privacy Form
  • Use the Immunization Upload on the left menu to upload all documents that verify that you have received all required vaccinations/immunizations
  • Submit your information

Acceptable verification of immunization:

  • Childhood Vaccination Card
  • Vaccination/Immunization records from your doctor’s office (signed and stamped with your doctor’s license number by your doctor or clinic)
  • Vaccination/Immunization Form (signed and stamped with your doctor’s license number by your doctor or clinic)
  • High school or prior college’s vaccination/immunization records
  • Blood test (original lab report is required)

All immunization records/forms and copies of laboratory reports must be submitted in English.  If your vaccine/immunization records are in a language other than English, please translate the vaccine name and date of each dose into English before uploading.

Health Insurance

Private Health Insurance Overview:

             In accordance with the Health Insurance Policy, all students must have adequate private health insurance coverage.  Students will be automatically enrolled in the university offered health insurance plan and charged in the fall and spring semesters.  The health insurance fee is dependent on when you enroll at Carnegie Mellon University, Qatar.  You can request to waive this insurance if you are able to provide documentation that you have alternative private health insurance that meets the minimum requirements.  All waivers need to be submitted by July 1st to be considered. 

If you are a student who is sponsored, you should verify with your sponsor whether they provide Private Health Insurance AND complete the Sponsor Private Health Insurance Form 

Required Private Health Insurance Information to waive the Student Health Insurance Plan:

Government Health Insurance Overview (required for all students):

             The insurance through the government is referred to as the Hamad Health Card.  The Hamad Health Card is required for all students and it offers subsidized optometry, dental and medical care in the public primary health care centers and the public hospitals.  CMU-Q will guide international students with understanding the process of obtaining a Hamad Health card once their Residency Permits are issued.  Local expatriate and Qatari Nationals should upload their Hamad Health Card to Health Connect (under messages) by July 1st.

When will I receive my Andrew ID?

You will receive your Andrew ID via the email address you used on your application to CMUQ within one week of paying your deposit.

I received the CMUQ Slate System Admission ID is this the same as the Andrew ID?

Your Admission ID is different from your Andrew ID. You will receive your Andrew ID via the email address you used on your application to CMUQ within one week of paying your deposit.

What are the Health requirements for students who are new to CMU-Q?

All full-time incoming students must complete the online immunization, as well as upload immunization records with dates from their healthcare providers. If your immunization records are in a language other than English, please translate the date of each vaccine dose into English before uploading.

These forms must be completed on Health Connect and documents verifying you completed the immunizations should be uploaded on Health Connect

  • Complete the online Health History form
  • Complete the online Immunization form
  • Read and acknowledge the online Notice of Privacy Form
What immunizations are required?

Refer to the New Student Information webpage and access the Required Immunizations document for additional information.

My Mum says I had Chicken Pox (Varicella) when I was young can I just note this on the vaccine page?

No, as the only accurate way to confirm that you had the disease or have been immunized is via a Varicella blood titer.

Where do I access the online forms and upload my records?

When you have your andrew ID, you can access all of our online forms in HealthConnect (select Forms). You can also upload your immunization records here.

Please note: in order to access HealthConnect, your birthdate in the Student Information System (SIO) must be correct. If you are unable to access HealthConnect, check your birthdate in SIO. If it is incorrect, correct it at the HUB. The following day, it should be updated in HealthConnect. If you still have problems, contact the Office of Health and Wellness.

I haven’t received my Andrew ID. What should I do?
  • Have you paid your deposit? Was it paid at least a week ago? If so, you will receive your Andrew ID via the email address you used on your application to CMUQ within one week of paying your deposit.
  • You should receive an email from Computing Services, it-help@cmu.edu that includes the FirstPass instructions for retrieving your Andrew ID and setting your password.  If you have not received this email, you can email helpcenter@qatar.cmu.edu and they will assist you in getting it set up.
